Ceramides Explained: The Lipids That Support Your Skin Barrier


“Healthy skin isn’t only about what you stimulate. Sometimes it’s about replacing what the skin needs to protect itself.”


Ceramides aren’t one of skincare’s flashiest ingredients.

They don’t promise dramatic exfoliation. They don’t create the immediate sensation we associate with acids.

And they aren’t usually marketed with the same excitement as Vitamin A or Vitamin C.
But biologically, ceramides are incredibly important.

They are naturally occurring lipids — fats — found within the outermost layer of your skin, where they work alongside cholesterol and free fatty acids to help create the organised lipid matrix of the skin barrier. 

Think of your skin barrier as a carefully constructed wall.

The skin cells — corneocytes — are often described as the bricks.

The lipids surrounding them are the mortar.

And ceramides are a major part of that mortar.

When this lipid structure is functioning well, the skin is better able to retain water, resist irritants and maintain a healthy protective barrier.

When it becomes disrupted?

Skin can start behaving very differently.

What Exactly Are Ceramides?


Ceramides are a family of naturally occurring lipids composed of a fatty acid joined to a sphingoid base.

Most of them are concentrated within the stratum corneum — the outermost layer of the epidermis.

Here they exist alongside two other major groups of barrier lipids:

Ceramides + cholesterol + free fatty acids

Together, these lipids organise themselves into highly structured layers between skin cells, helping create the water-resistant barrier separating our body from the outside world. 

And this is an important distinction:

Ceramides aren’t simply moisturising ingredients we’ve invented for skincare.

Your skin naturally contains them.
Topical skincare attempts to support a structure that already exists within healthy skin.

Why Is the Skin Barrier So Important?

Your skin barrier has two particularly important jobs:

Keep the right things in.

And:

Keep the wrong things out.

It helps reduce excessive water loss from the skin while limiting the penetration of environmental irritants and other external substances.

When the barrier becomes disrupted, water can escape more readily through a process called transepidermal water loss — TEWL.

The result can be skin that feels:

  • dry
  • tight
  • rough
  • flaky
  • sensitive
  • itchy
  • irritated
  • uncomfortable

A compromised barrier can also make skin more reactive to skincare products that were previously well tolerated.

That’s why someone may suddenly say:

“Everything I put on my skin stings.”

The problem isn’t necessarily that every product has suddenly become unsuitable.

Sometimes the environment those products are being applied to has changed.

The “Bricks and Mortar” Analogy


This is one of the simplest ways to understand ceramides.

Imagine a brick wall.

The bricks - the corneocytes — flattened cells forming the outermost layer of skin.

The mortar - the organised mixture of ceramides, cholesterol and free fatty acids surrounding those cells.

If the mortar becomes deficient or disorganised, the wall becomes less effective.

More water can escape.

External irritants can penetrate more easily.

And the skin may become increasingly reactive.

Ceramides therefore don’t simply make skin feel moisturised.

They participate in the physical architecture of the skin barrier itself. 

What Happens When Ceramides Are Reduced?

Lower ceramide content and changes in the composition or organisation of barrier lipids have been associated with dry, barrier-disrupted skin and conditions including atopic dermatitis. 

But you don’t need to have eczema to experience a compromised barrier.

Everyday factors can disrupt barrier function too.

These may include:

  • excessive cleansing
  • repeated exfoliation
  • overuse of acids
  • strong retinoids
  • environmental exposure
  • low humidity
  • very hot water
  • some professional treatments
  • ageing
  • inflammatory skin conditions

Often we see several factors occurring at once.

Someone might use a retinoid, exfoliating acid, foaming cleanser and another active serum — while assuming the resulting dryness means they simply need more hydration.

Sometimes they do.

But hydration and barrier lipids aren’t the same thing.

Ceramides vs Hydration: What’s the Difference?

This distinction is important.

Ingredients such as hyaluronic acid and glycerin primarily act as humectants, helping attract and hold water.

Ceramides are lipids. Their role is much more closely connected with the structure and organisation of the barrier.

So, while Humectants help provide or retain water.

Barrier lipids help create the structure that limits excessive water loss.

A good moisturising formulation can contain both.

This is why simply layering more hyaluronic acid onto chronically dry, compromised skin isn’t necessarily enough.

The skin may need water + lipids + protection from further disruption.

Do Ceramides Actually Repair the Skin Barrier?


This is where we need to be careful with skincare language.

“Barrier repair” is used very freely in marketing.

Ceramide-containing formulations have evidence supporting improvements in hydration and aspects of barrier function, particularly in dry or barrier-impaired skin. 

A 2025 study in adults predisposed to atopic dermatitis found that a moisturiser containing physiological lipids improved barrier integrity and reduced sensitivity, while the comparison moisturiser improved dryness but didn’t produce the same barrier-function changes. 

However, this doesn’t mean: ceramides + any cream = automatic barrier repair.

The complete formulation matters enormously. Which brings us to something I think is particularly important for this article.

Not All Ceramide Products Are Equal


Seeing the word CERAMIDE on the front of a moisturiser doesn’t automatically tell you how effective that formulation will be.

Ceramides need to be incorporated appropriately into a formulation.

Their type, concentration, accompanying lipids and physical organisation can influence how a product behaves.

A 2024 scientific review specifically highlighted the importance of correct ceramide formulation and reported that poorly incorporated or undissolved ceramides can compromise the intended barrier-repair performance of a formula in experimental models. 

So just as we’ve discussed throughout Volume Three:

Ingredient name ≠ formulation quality. A beautiful ingredient can still exist within an average formulation.

Ceramides Don’t Work Alone


This is another skincare simplification worth correcting.

Your natural barrier isn’t made entirely from ceramides.

Its major lipid classes include:

Ceramides - Important structural components of the intercellular lipid matrix.
Cholesterol - Another important component of normal barrier organisation.
Free Fatty Acids - Also contribute to the structure and function of the lipid barrier.

These work together.

That’s why some barrier-supportive skincare formulations deliberately contain combinations of ceramides + cholesterol + fatty acids, rather than treating ceramides as an isolated hero ingredient. 

The exact formulation matters more than simply checking whether “ceramide” appears somewhere on the ingredient list.

Are There Different Types of Ceramides?

Yes — and this gets surprisingly complicated.

Ceramides aren’t one single molecule.

Human skin contains numerous ceramide subclasses with different molecular structures.

On cosmetic ingredient lists, you may see names such as:

  1. Ceramide 

  2. Ceramide NP 
  3. Ceramide EOP
  4. 
Ceramide NS
  5. 
Ceramide AP
  6. Ceramide AS

You may also encounter synthetic ceramide-like molecules or pseudoceramides designed to perform similar barrier-supportive functions.

For the average consumer, memorising every ceramide abbreviation isn’t particularly useful.

A longer list of ceramide types on the label doesn’t automatically mean the product will perform better.

Once again, look at the formulation, not simply the ingredient count.

Who May Benefit From Ceramides?


Ceramide-containing skincare can potentially be useful across many skin types.

Dry Skin - especially when dryness is accompanied by roughness, flaking or tightness.

Dehydrated Skin - ceramides can complement humectants by helping support the barrier responsible for reducing excessive water loss.

Sensitive or Reactive Skin - supporting the barrier may help improve resilience, although sensitive skin can still react to other ingredients within a formulation.

Ageing Skin - barrier function and lipid composition can change with age, making barrier support an important — and often overlooked — part of healthy-ageing skincare.

People Using Vitamin A - ceramides can be particularly useful within retinoid routines where dryness and irritation need to be carefully managed.

People Using Exfoliating Acids - again, the objective isn’t to counteract deliberately excessive exfoliation. It’s to maintain a healthy barrier while using actives appropriately.

Eczema-Prone Skin - altered ceramide composition is well documented in atopic dermatitis, and moisturisation/barrier support forms an important part of management. 

However, eczema is an inflammatory medical condition and skincare isn’t a replacement for appropriate medical treatment. 

Ceramides and Sensitive Skin

This deserves its own section.

People with sensitive skin often focus on finding products labelled: “gentle”
“calming”
“for sensitive skin”

But sensitivity isn’t always simply about avoiding irritating ingredients.

Sometimes the underlying issue is barrier vulnerability.

If the barrier isn’t functioning optimally, substances can penetrate differently and nerve endings may become more easily stimulated.

That’s why barrier support can be an important part of managing reactive skin.

But there’s an important caveat:

A product containing ceramides can still irritate your skin.

The complete formulation may also contain fragrances, botanical extracts, preservatives or active ingredients that aren’t appropriate for you.

Ceramides don’t automatically make every formula gentle.

What Works Well Alongside Ceramides?


Ceramides are exceptionally easy to incorporate into a broader skincare routine.

Niacinamide - A particularly logical combination. Niacinamide can support processes involved in endogenous ceramide production, while topical ceramides can provide barrier lipids within the formulation. 

Hyaluronic Acid - Provides complementary hydration.

Glycerin - Another excellent humectant that helps attract and retain water.

Fatty Acids + Cholesterol - These naturally coexist with ceramides within the stratum corneum and can be useful components of well-designed barrier formulations.

Vitamin A - Ceramides can help support the barrier within a carefully managed retinoid routine.

Peptides
Can comfortably coexist with ceramides in healthy-ageing routines.

PDRN / Polynucleotides + Exosome-Based Skincare
These ingredients occupy different areas of emerging regenerative skincare.
There isn’t good evidence to say ceramides specifically make PDRN, polynucleotides or exosomes “work better.”

However barrier-supportive skincare may form part of a broader routine where regenerative ingredients are also being used.

Ceramides vs Occlusives

These are also sometimes confused.

An occlusive ingredient primarily works by forming a layer over the skin that reduces water evaporation.

Examples include petrolatum and certain oils or waxes.

Ceramides work differently because they are physiologically relevant components of the skin’s intercellular lipid structure.

Both approaches can reduce moisture loss, but through different mechanisms.

And sometimes an intelligently formulated moisturiser uses:
humectants + emollients + barrier lipids + occlusive ingredients
together.

Skincare doesn’t always need one “hero ingredient.”

Often the formulation works precisely because several components are doing different jobs.

Can Ceramides Clog Pores?


Ceramides themselves aren’t automatically comedogenic.

But — just like our discussion about niacinamide — you don’t apply isolated ceramide molecules to your face.

You apply a complete moisturiser.

Some richer barrier creams may contain oils, butters, waxes or other emollients that don’t suit every acne-prone skin.

So if someone experiences congestion from a particular ceramide moisturiser, it doesn’t necessarily mean:

“My skin can’t tolerate ceramides.”

It may simply mean that particular formulation isn’t appropriate for their skin.

Do Oily Skins Need Ceramides?

Potentially, yes.

Oil production and barrier health are not the same thing.

Someone can have abundant sebum and still have a compromised skin barrier — particularly if they’re aggressively cleansing, exfoliating or treating acne.

This is one of the most common mistakes we see with oily skin.

Oil appears → skin is stripped → skin becomes irritated → more acne actives are added → barrier becomes increasingly compromised.

The answer isn’t necessarily a heavy barrier cream.

But it may be a lighter formulation containing appropriate barrier-supportive ingredients.

Oily skin still needs a functioning barrier.

Can You Overuse Ceramides?


Ceramides aren’t active ingredients in the same sense as retinoids or exfoliating acids, so there isn’t usually a need to “cycle” them.

They can generally be used:

AM and/or PM
 daily
alongside most active ingredients
depending on the formulation and individual skin.

But that doesn’t mean everybody needs a separate ceramide serum, ceramide moisturiser and ceramide mask.

If your moisturiser already provides adequate barrier support, adding several more products may simply add complexity.

This brings us back to one of our recurring themes:

More skincare isn’t automatically better skincare.

When Barrier Repair Requires More Than Ceramides

This is important.

If your skin barrier is repeatedly becoming irritated, the solution isn’t necessarily to keep your aggressive routine unchanged and simply add a ceramide moisturiser over the top.

You may need to reassess what’s causing the disruption.

That might mean temporarily reducing:

  • exfoliating acids
  • physical exfoliation
  • high-strength retinoids
  • harsh cleansing
  • unnecessary active layering

Ceramides can support the barrier.

They don’t give us permission to continually damage it.

And persistent redness, itching, cracking, weeping, rash or recurrent dermatitis warrants appropriate professional or medical assessment rather than endless skincare experimentation.

For years, skincare marketing has focused heavily on stimulation:

Exfoliate more.

Turn cells over faster.

Use stronger acids.

Increase percentages.

Stimulate collagen.

But healthy skin also requires protection, hydration and structural integrity.

At The Skin Code, when we assess someone’s routine, we don’t only ask:

“Which active ingredient should we add?”

We also consider:

1. Is the skin barrier functioning well?
2. Is the skin retaining moisture effectively?
3. Is the routine removing too much lipid from the skin?
4. Are active ingredients being used more aggressively than the skin can tolerate?
5. Does the moisturiser provide adequate barrier support?
6. Would simplifying the routine actually produce a better result?

Sometimes improving a skincare routine isn’t about adding another active.
Sometimes it’s about rebuilding the foundation that allows everything else to work properly.

“A strong skincare routine doesn’t begin with the strongest active. It begins with a skin barrier capable of tolerating what you ask it to do.”

Dryness, dehydration, sensitivity and barrier impairment can look very similar — but they aren’t necessarily the same thing.

A professional skin assessment can help identify what’s happening within your skin and whether your routine needs more hydration, greater lipid support, fewer active ingredients — or simply less intervention.

Because sometimes the smartest skincare decision isn’t adding more.
It’s restoring what the skin needs to function well.
